The Heart Failure Clinic at CVAM provides specialized care and education for patients diagnosed with heart failure, helping them to understand their condition and how to optimally manage their health. With enrollment in the heart failure clinic, we strive to help patients control their heart failure, reach personalized goals, and avoid hospitalizations.

We work one-on-one with patients and their caregivers and coordinate closely with their cardiologist, who will be fully involved in their care. With the right care, heart failure is manageable and will not stop the patient from doing the things that they enjoy. The prognosis for heart failure depends on how well the heart muscle is functioning, heart failure symptoms, and the patient’s response to the treatment plan.
