Renew a Prescription

Prescription renewals are handled 9:00 AM to 4:00 PM, Monday through Friday. Please allow at least 72 hours to accommodate your request, as each request has to be approved by a physician and all physicians are not in the office every day.

If your prescription is running low, please do not wait until after office hours, the weekend or until your medication has completey run out. If you are planning to travel out of town, please make sure you have enough medication to take on your trip.

Now you can request a prescription refill at any hour of the day or night, at your convenience, though our patient portal.

Renew by Fax

Please download and fill out the Prescription Renewal Form, then fax it to (480) 218-4353.

Renew by Phone

Call us, during office hours, at (480) 641-5400 to request a refill. Please be prepared with the following information:

• Name of medication
• How it is taken?
• Do you want a 30-day or 90-day refill?
• Phone number for your pharmacy
