Register as a New Patient
Welcome to CardioVascular Associates of Mesa. We are pleased to be your provider of cardiac, peripheral vascular and electrophysiology care.
Our goal is to provide you with compassionate patient care and service. We invite you to begin your new patient registration now by following these directions:
1. Fill out the new patient forms we mail you
We will send you a new patient registration packet in the mail. Please fill out all of the forms and bring them with you to your first appointment!
Forms will include:
- Welcome letter with an overview of instructions
- Appointment itinerary
- Patient profile
- *Medical records release form (to complete and forward to your primary care physician's office to send us your records)

2. Review the forms below and bring them to your appointment
Please print the financial policy acknowledgment form, complete it and bring it to your appointment. Please review the HIPAA policy. Then, please print the HIPAA acknowledgment form, complete it and bring it to your appointment.
Please arrive at least 30 minutes early, if you have not yet filled out these forms. If you have completed forms with you, you only need to check in with the reception desk 15 minutes before your scheduled appointment, so you can complete history and current symptoms forms.

Have your primary care physician and/or your referring physician fax your records to (480) 218-4353. If you have your records, please bring them to our office at least one week prior to your appointment so your physician can review them and prepare for your visit.
Failure to have medical records available at your first appointment may result in cancellation of your appointment.
Send us your referral
If your insurance plan requires a referral, please have your referring physician fax it to (480) 325-1390. If they already provided you with a paper referral, please bring the referral to your appointment. If you are arriving for testing only, please bring the physician order to your appointment.
Failure to have a referral available at your first appointment may result in cancellation of your appointment.
Prepare for your office visit
In addition to completing the forms, we will also require the following for your first visit:
- Driver's license or other valid government-issued photo identification
- Insurance card
- Any co-pay, deductibles or co-insurance applicable to your health plan
- Prescription medication bottles, as well as any vitamins and supplements
